I need a php script built and installed that runs on a mysql database. (I do have phpmyadmin) and cpanel.
THE USER’S FORM PAGE
This script is rather simple it is basically a form that people fill out. I have attached a screen shot of the fields needed. Basically all the fields will be dropdown fields. except the name field, which will be a text field, the date/year field, which will be a text field, quantity of game, which will be a text field, and the Review field, which will be a text box to write paragraphs in. For the drop downs I will give you specific static field entries for the drop downs.. there maybe around 5-8 choices for each dropdown. Also the default entry in the drop downs would just say “SELECT ONE” and that would be disallowed.
The “how may we contact you” field will actually give 2 fields, Email (text field) and phone (text field) If both fields are left empty the output page would say this hunters does not want to be contacted. The output page would show it as a url
All fields would be required except the Name field, and the Contact fields. Again, if they are left blank the output page would say in place of their name “Anonymous”. and if the contact info is blank the output page would say that this hunter does not want to be contacted.
All files for this script should be in a folder called /outfitter-reviews/
the admin folder should be located in /outfitter-reviews/admin
this way I can password protect the folder on the server
How this will work is that the user fills out the outfitter review form on /outfitter-reviews/form.php. When they hit submit it emails this review to the admin email (my email). I get an email with a link to the admin panel where I can see the list of emails that need to be approved. From there I can delete or edit their review and then approve it to be made visible on the output page located at /outfitter-reviews/index.php
Only approved reviews will be visible for the public on /outfitter-reviews/index.php (the Output Page)
I will need to be able to delete or edit even approved reviews at anytime in the admin as well.
Also I want a order feature so that I can change the order of the reviews.. In other words when approving a review I would choose from a dropdown something like:
At the TOP
Below Review #1
Below Review #2
etc..
The form needs to use capcha to help fight spam.
THE OUTPUT PAGE
It will simply list the the reviews down the page.. (20 per page before going to page 2 and 3 and so on)
This will need to have links at the top of the page that show links to the list of location fields.. then if someone click on one of those links it would only show reviews for that location.. I would need the same thing for species hunted.. If these fields fields need to be added dynamically to make these work I am ok with that. In other words the admin would have a “add a location” “edit a location” and “delete a location” as well as “add” “edit” and “delete” a species.
I want to make sure this code is easy to change.. I have worked with programmers in the past that make it difficult for other to edit thier code by encoding things. I don’t want that! No design is needed orther then some text formatting usign a css file for the page.. Also Please apply a “top.php” and a “bottom.php to the output page so that I can easily change the design for that page.
I am looking for someone who knows what they are doing.. No beginners wanted. I want someone who can understand English and uses MSN Messenger so that we can communicate during the process. Please be honest about your completion dates. I am looking for someone who can keep me informed of their progress.
